. All forms must be completed with the required documents and minimum payment of $150 received by the Membership Director before your child is allowed to begin practice. If registering online, you must also submit the required documents (listed on the checklist in the Registration Packet) and minimum payment of $150 received before your child is allowed to practice. Full payment is due no later than April 15th.
